Notes go in the large right-hand area during the lecture. Afterwards you fill the cue column with the questions those notes answer. Later still, you write the summary at the foot from memory, and revise by covering the notes and answering the cues.
The point is that reviewing is built into the page. A sheet of ordinary notes gives you nothing to test yourself against; a Cornell sheet turns the same content into a set of questions with the answers hidden beside them.
Every band is adjustable, because how much room each needs depends entirely on the subject.

Usually not. Cues are short phrases rather than sentences, and leaving the column blank makes it visually distinct from the notes. The option is there if you prefer it ruled.
