A3 is exactly two A4 sheets side by side, so a grid at the same spacing gives you roughly twice as many squares in each direction — four times as many in total. For a plot that was running off the edge of A4, that is usually the whole solution.
It is also the smallest size at which a fine grid stays useful across a whole drawing. A 1 mm grid on A3 gives nearly 300 by 420 divisions, which is a serious plotting surface.

Yes. The PDF carries the exact 297 × 420 mm page size, so a plotter driver has no reason to rescale it. Choose Actual size in the driver as usual.
Only by scaling it down, which changes the grid size. If you need a large sheet from a small printer, print two A4 sheets and tape them.
