A PDF can declare its own page dimensions, and this one declares 210 × 297 mm. That matters because a viewer or printer driver that knows the page is already A4 has no reason to scale it to fit.
A sheet exported as Letter and printed on A4 gets shrunk by about 3% to fit the narrower page — enough to turn a 5 mm grid into 4.85 mm. Starting from the right page size avoids the problem instead of correcting for it.
